Our Services
Edenbridge PACE provides a range of services twenty-four hours a day, seven days a week, three hundred sixty-five days a year.

Eligibility criteria
To be eligible for Edenbridge PACE care, the participant must meet the following qualifications:
- 55 Years of age or older
- Qualify for nursing facility level of care
- Live within our service area
- Live safely within your community
By choosing to enroll in Edenbridge PACE, participants must receive all needed healthcare services, including primary care and specialist physician services (other than emergency services) from Edenbridge PACE or a person or company pre-authorized by Edenbridge PACE. Edenbridge PACE guarantees access to services, but not to a specific provider.
